I am completing a form 2848, but i am not sure about part 2?

person A is incarcerated and she has elected person B to be her power of attorney. person B does not fit into any of the categories a-h, what do i do? person B has the power of attorney agreement with both A's siganture and B' signature. would this agreement be enough to suffice or does part 2 need to be filled out? there is also a portion that the incarcerated needs to fill out but they are unable to because that are incarcerated. what needs to be done?
